Required Qualifications
Bachelor's degree in Social Work, Psychology, Sociology, Public Health, Nursing, or a related field. A minimum of one (1) year of experience working with patient populations receiving medical care or social services.
Preferred Qualifications
Experience in care coordination and/or case management. Experience with health IT systems and reports.
